Chronic Care Management 

Managing your health can feel overwhelming, but you don’t have to do it alone.
That’s why we’re here.

Chronic Care Management, or CCM, is a program for Medicare patients, supported by Carolina Pines Medical Group, that gives you regular support outside of your normal doctor visits. Our team helps with things like keeping track of medicines, answering health questions, and staying connected with everyone involved in your care – like your doctors, pharmacy, insurance, transportation, etc.

 

Am I eligible for the CCM Program?

If you have Medicare and one or more chronic conditions, Carolina Pines Medical Group can help you better manage your health. If you’re unsure whether your conditions qualify, please reach out to your primary care provider or our Chronic Care Management team.

Examples of chronic conditions include, but are not limited to:

  • Alzheimer’s disease and related dementia
  • Cancer
  • Cardiovascular disease & other heart conditions
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Depression
  • Diabetes
  • Hypertension
  • Kidney disease
 
 

What’s Included in the CCM Program?

Your CCM team at Carolina Pines Medical Group will coordinate a number of services on your behalf, keeping you informed and cared for. They will assist you in several ways, including:

  • Attending to any of your medical care needs between visits with your primary care provider (PCP).
  • Working with your PCP to create a care plan that revolves around your personal health goals. 
  • Providing guidance in establishing new care and utilizing resources within your community.
  • Keeping you up to date on upcoming preventive services and immunizations. 
  • Advising you and keeping you informed on your medical conditions and the medications your doctor prescribes.
  • Ensuring priority access to your PCP for any urgent needs.
 
 

Why Should You Join a Chronic Care Management Program? (CCM)

According to the CDC, three in four Americans 65 years and older have multiple chronic conditions that require ongoing care, and about 1 in 10 hospital stays can be prevented with better care management.

With the help of a personal care coordinator, Carolina Pines Medical Group works with you every month to better manage chronic medical conditions. With access to our CCM support team, our program can help you save time, money, and the worry that comes with unexpected medical problems.
